Under the supervision of the Trans Health Program Manager, the Youth and Family Care Coordinator supports the care of trans, nonbinary, and gender expansive youth by (1) providing low-barrier informational and supportive services about trans and gender diverse (TGD) youth and their care to current and prospective patients, families, and community members; (2) supporting the care of young TGD patients through administrative duties and care coordination; and (3) serving as a centralized point of contact for Fenway staff, patients, families, and community members regarding TGD youth care and services.
Representative Duties:
- Provides direct services to current and prospective patients and families:
- Assists patients and community members in accessing medical and social services such as primary care, behavioral health care, legal advocacy, and community resources
- Provides on-going support to patients and families as appropriate
- Maintains designated open community office hours for low-barrier access to information about resources and services
- Spearheads, organizes, and leads programming and community engagement efforts such as community events and information sessions focused on TGD youth and families
- Conducts outreach to help retain patients in care
2. Supports internal staff and providers :
- Assists providers, case managers, and other health center staff with locating specific resources, information, and referrals as requested.
- Collaborates on maintaining shared knowledge base, insurance information, referral resources, and related information for staff and provider use.
- Develops and maintains workflows as needed to support providers in administrative aspects of TGD youth patient care, including coordination with external medical and behavioral health providers
- Assists in management of specialty medications, including tracking prior authorizations, medication delivery status, and other related care coordination for TGD patients accessing these medications
- Attends and participates in all meetings regarding the care of TGD youth works closely and in conjunction with patient services, medical and BH care teams to complete wrap-around supports for youth and families in establishing care at Fenway
3. Supports Trans Health Program Operations:
- Provides administrative support to Trans Health Program activities, including scheduling, meeting minutes, and meeting management.
- Collaborates in creation and maintenance of resource listings appropriate for trans and gender diverse youth and families
- Participates in community councils, boards, and groups as appropriate to further programmatic objectives
- Consults with a variety of health centers and community staff about specific patients and general approaches to patient treatment and care coordination
4. Performs and completes administrative tasks in a timely and efficient manner:
- Completes appropriate documentation for each patient interaction
- Participates in staff and team meetings and conferences, and other THP activities as appropriate
- Collects, prepares, and reports monthly on utilization, demographic, etc. for all supports and services provided, planned, and executed
5. Meets agency participatory expectations:
- Suggests and participates in continuing quality improvement efforts
- Adheres to all agency and departmental policies and procedures
- Participates in quality assessment and improvement activities as requested
- Adheres to the highest principles of patient and client confidentiality
- Adheres to established safety policies, procedures, and precautions; identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation
- Attends all required meetings, in-services, and professional trainings
- Maintains professional competence necessary to perform job responsibilities; maintains and provides agency with records of continuing education activities

Requirements
- Excellence in working with transgender and gender diverse people
- Knowledge of MS Office suite, particularly Excel and Outlook
- Demonstrated ability to take responsibility for assignments, prioritize tasks, meet deadlines, and to work as part of a team
- Outstanding communication (written and verbal) skills
- Highly attentive to detail, highly organized, and deadline-oriented
- Skilled at managing confidentiality and boundaries in clinical, educational, and community work
- Ability to work harmoniously with diverse groups of individuals
- Available to work some evening and occasional weekend hours
Preferred/Other Experience:
- 2+ years clinical administrative experience preferred
- Experience with clinical information, terminology and operations is highly desirable
- Knowledge of Boston-area supports and resources for TGD youth and families
- Demonstrated experience working in a multi-cultural environment where commitment to diversity based on race, age, ethnicity, sexual orientation, gender identity, and physical ability is an institutional and programmatic value
